Please Note the following:

Medical Record requests require a $25.00 copy fee for each medical record requested. The copying of the record can take form 2-3 weeks depending on if we need to obtain materials from other medical offices.

Physical forms as needed for companies and schools are the patient’s responsibility to bring to us to complete. These may take up to 2 weeks to complete.

Disability and Medical Equipment forms are usually issued by your insurance carrier and are your responsibility to obtain for us to complete. These forms may take 4-6 weeks to complete.

Medical Referrals are required by your insurance companies. Please call our main phone number, choose the option for referrals and follow the prompts to request a medical referral. Referrals must be requested at least 72 hours prior to the visit.

Prescription requests should be done before you have completed your chronic medications. Please call our main number and choose option for prescription requests, follow the prompts. Prescription requests may take 1-2 days to complete depending on whether a prior authorization is required. Narcotic prescriptions cannot be called into the pharmacy; they require you to pick up a written prescription from the office.

Billing concerns: You may call our main number and choose the option for billing issues, or you may contact us here: https://373.portal.athenahealth.com with your billing concern. Your concern will be addressed within the week. You may be contacted by phone or e-mail with a response. In some cases a call back is not needed.

Bounced Checks will incur a $10.00 bank fee.

Payment Plans are available if you have difficulty paying your bill. We make every effort to accommodate our patients but in the event that our relationship is ineffectual; our office reserves the right to terminate services with patients as we deem necessary.
